In order to secure your spot in the program, you'll need to complete all required acceptance forms and pay the non-refundable program fee.
Your spot in the program is not secured until all required forms and information have been submitted AND the non-refundable program fee is paid. (Including the I-20 processing fee for international students needing a Stanford-sponsored I-20.)

| Medical Information & Proof of Insurance |
Acknowledge Cardinal Care Health Insurance Enrollment and option to waive. Provide medical information including medications currently being administered. If available, provide current insurance coverage information. This information is for Summer Session reference only and is not for waiving Cardinal Care. There will be a separate process to waive Cardinal Care after your spot in the program is secured. |
| Emergency Contact Information | Provide emergency contact information. |
| Local Contact (Commuter only) | Commuters are expected to live within 50-mile radius of campus. Summer Session will verify local contact to ensure students are staying within commuting distance with a trusted friend or family member who is over 18 years of age. |

| Copy of Passport (International Students Only) | Provide a copy of your passport clearly displaying all information. Your passport must be valid through November 1, 2024 |
